Volunteer Policy
Quakertown Youth Baseball Association takes the safety of our youth very seriously. Therefore, we follow all State, Federal and Organizational laws regarding safety and volunteer background checks.
Anyone that comes in regular contact with our players (either in the dugout at games or on the field at practice) will adhere to this policy.
Until these obligations are fulfilled, no adult may participate in team activities.
The QYBA Board cannot direct you in this process. All of our collective knowledge is sourced from the information below.
Act 24 Arrest and Conviction Disclosure Disclaimer
In accordance with Pennsylvania Act 24 of 2011, all volunteers, employees, and individuals who have direct contact with children through the Quakertown Youth Baseball Association (QYBA) are required to disclose any arrest or conviction that occurs after the date of their most recent background clearance.
By volunteering with QYBA, you acknowledge and agree to the following:

You are required to report any arrest or conviction for an offense listed under Act 24 within 72 hours of the occurrence.

This disclosure must be made in writing to QYBA leadership. You may fill out this FORM and mail it to the board.

Failure to disclose an applicable arrest or conviction may result in immediate suspension or removal from volunteer duties and may affect your eligibility to continue participating in the program.

QYBA reserves the right to review all disclosures and take appropriate action in accordance with Pennsylvania law and league policy.
This requirement is in place to ensure the safety and well-being of all children participating in QYBA programs and events.
STEP 1: PENNSYLVANIA BACKGROUND CHECK REQUIREMENTS
Be sure to electronically save the following three documents as you receive them.
1: PA Child Abuse History Clearance (free):
•Click Here to access the form. Select the link for an “Individual Login” or “Create Individual Account” if you do not have one.
2. PA Criminal Record Check (free):
• Click Here to access the form. Select “New Record Check Volunteers Only”. If there is no record, you will receive a downloadable document indicating “no record.” If there is a criminal record, your results may take a couple weeks.
3: Disclosure Statement or FBI Fingerprint Clearance: One of these options must be completed even if clearances came from another organization:
Option 1: If you have been a resident of the Commonwealth of PA for AT LEAST the past 10 YEARS, complete, sign and submit the “Disclosure Statement for Volunteers” form which can be downloaded and printed by Clicking Here.
Option 2: If you have NOT been a resident of the Commonwealth of PA for AT LEAST the past 10 YEARS, you will be required in accordance with PA Law to obtain an FBI federal background clearance (Criminal History Record Information (CHRI)) which involves fingerprinting. By completing the above requirements, you will have a clearance to volunteer with children in Pa. for five years.
